Address Change Request Form

Please fill out the form below, and someone will reach out to you to finalize your address change.

In order to finalize and execute an address change, you must also supply us with two proofs of residency and a photo ID. All communication, including proofs of residency will go to rgarrett@chestercommunitycharter.org.

PLEASE READ THE FOLLOWING BEFORE SUBMITTING ADDRESS CHANGE DOCUMENTS:

ALL DOCUMENTS MUST BE IN THE NAME OF A PARENT OR GUARDIAN AND MUST BE CURRENT.  Expired leases and utility bills older than 60 days are NOT acceptable.

The first THREE (3) documents listed are REQUIRED BY THE STATE OF PENNSYLVANIA to change a student address:

  1. EITHER A LEASE OR DEED – if you live in Delaware County and can’t find your deed, we have access to print if for you

  2. UTILITY BILL – PECO, WATER OR SEWAGE

  3. PHOTO ID

  4. NOTARIZED AFFIDAVIT OF RESIDENCY – this document is only necessary if you reside with someone else and your name is not on the deed or lease.  If you cannot get to a notary, we have one in our office Monday through Friday from 7:45 a.m. – 12:00 p.m. and 1:00 p.m. – 4:30 p.m. who can complete this document with you

  5. ECYEH INTAKE FORM – this document is only necessary if you are currently homeless or temporarily displaced

  6. SHELTER or AGENCY LETTER – this document is only necessary if you are currently staying in shelter or working with an agency for placement.  Please note, in addition to this, you will also need to complete the ECYEH INTAKE FORM
